Description: This includes information related to a healthcare services support request (e.g. in relation to providing Family Support Services, Recovery Coordination Program services, assessments and evaluations for an individual or group of people, etc.). The reasons for this request may include (but not be limited to) healthcare support for a recovering service member (RSM) or a victim of abuse. The request is made to the Department of Defense (DoD) healthcare entity and may include (but not be limited to) information such as the requestor name, request date and explanation of the request.
